Abstract
Ketamine, a known antagonist of N-methyl-D-aspartic (NMDA) glutamate receptors, had been used as an anesthetic particularly for pediatric or for cardiac patients. Unfortunately, ketamine has become an abusive drug in many parts of the world while chronic and prolonged usage led to damages of many organs including the brain. However, no studies on possible damages in the brains induced by chronic ketamine abuse have been documented in the human via neuroimaging. This paper described for the first time via employing magnetic resonance imaging (MRI) the changes in ketamine addicts of 0.5–12 years and illustrated the possible brain regions susceptible to ketamine abuse. Twenty-one ketamine addicts were recruited and the results showed that the lesions in the brains of ketamine addicts were located in many regions which appeared 2–4 years after ketamine addiction. Cortical atrophy was usually evident in the frontal, parietal or occipital cortices of addicts. Such study confirmed that many brain regions in the human were susceptible to chronic ketamine injury and presented a diffuse effect of ketamine on the brain which might differ from other central nervous system (CNS) drugs, such as cocaine, heroin, and methamphetamine.

Results: Cognitive impairment was found only in current ketamine users in the domains of mental and motor speed (p < .001), visual and verbal memory (p < .001), and executive functions (p < .001). Depressive symptoms were also more frequently found in current ketamine users (p < .001).
Conclusion: Current ketamine use is associated with cognitive impairment. Illicit substance treatment and rehabilitation services should pay attention to ketamine's cognitive effects and motivate their clients to quit using ketamine and stay abstinent.

Ik vind de bovenstaande studies zeer verontrustend. Ketamine was een van mijn favorieten drugs, echter gezien steeds meer studies bij langer termijn gebruik schade en/of cognitive problemen vaststellen stop ik voorlopig met gebruiken. De vraag die helaas niet in deze studies beantwoord word is: Wat is het effect van recreatief/sporadisch gebruik? De meeste studies gebruiken personen die langdurig elke dag gebruiken. Wat de invloed is bij recreatief gebruik met pauzes er tussen is dan ook nog maar de vraag.

Speculatie
De studie: Brain damages in ketamine addicts as revealed by MRI (2013), is mogelijk (alhoewel erg voorbarig gesteld) een van de eerste studies die Olney's lesions - NMDA receptor antagonist neurotoxicity (NAN) in mensen aantoont na gebruik van Ketamine. Een enkele studie is echter te vroeg om dit met zekerheid te stellen.

Volgens de Wikipedia pagina en aantal artikelen die ik heb gelezen over Ketamine word vaak benzodiazepine additioneel toegediend om Olney's lesions schade te voorkomen alsmede ter additionele verdoving. In hoeverre dit aansluit op preventie van de vastgestelde schade in de eerste studie durf ik ook niet te zeggen. Ik heb de combinatie zelf weleens getest, alleen dient zeker opgemerkt te worden dat veel gebruik van benzodiazepines ook een negatief effect heeft op geheugen. Daarnaast kan de combinatie ervoor zorgen dat je delen van de Ketamine trip vergeet.

Prevention
In medical settings, NMDA receptor antagonists are used as anesthetics, so GABAA receptor positive allosteric modulators are used to effectively prevent any neurotoxicity caused by them.Drugs that work to suppress NAN include anticholinergics, benzodiazepines, barbiturates and agonists at the alpha-2 adrenergic receptor in the brain, such as clonidine.

Deze studies zijn inderdaad best verontrustend, al zeggen zo een studies eigenlijk niets over recreatief gebruik. Op zich is elk langdurig en regelmatig gebruik van elke drug slecht. Het is dan ook vrij logisch dat je zo een resultaten kan krijgen bij mensen die ketamine hebben misbruikt. Maar het blijven natuurlijk geen leuke resultaten.

Of ik heb een déja vu, of ik heb dit daadwerkelijk eerder gezien...
In ieder geval: Duh, natuurlijk zal ernstige schade optreden bij dagelijks gebruik van drugs.
Ik schrik er niet van...

Het rapport zegt over het gebruik van de onderzochte (ex) ketamine verslaafden:

"The dosage used by the patients was from 0.2 to 3 g a day but majority dosage was 1 g a day. Among them, 19 of these patients took ketamine daily, while only two took it twice or three times a week. The durations of drug addiction ranged from 0.5 to 12 years".

Ze hebben het dus over relatief zware gebruikers. Ikzelf kom aan een keer of 6-8 per jaar (ong.300-350 mg per keer)

Ik ben het niet helemaal eens met de vanzelfsprekendheid die geuit word over hersenschade als gevolg van veelvuldig gebruik van bepaalde drugs. Er is namelijk een verschil tussen de cognitieve invloed van veelvuldig drugs gebruik en daadwerkelijke hersenschade zoals vertoont in de MRI studie. Het is zeker waar dat een aantal populaire drugs bij veelvuldig gebruik leiden tot hersenschade, echter om dit te generaliseren naar alle drugs ben ik het niet mee eens :geek: Het veelvuldig gebruik van THC bij volwassenen leid zover als ik bekend ben met de literatuur niet tot hersenschade, terwijl cognitie wel degelijk beïnvloed kan worden.

Uitzonderlijke is ook dat Ketamine in lagere doseringen neuroprotective en neurogenerative effecten heeft.

Bedankt voor je post deze studie had ik nog niet gezien. Deze studie lijkt een verdere bevestiging voor de bevindingen uit de eerste studie. Er word namelijk een reductie van grey matter vastgesteld in de prefrontale cortex, dit is onderdeel van de frontale lobes van de hersenen waarbij in de MRI studie ook cortical atrophy gevonden word. Cortical atrophy houd in dat er een verlies van hersencel en hersencel functie plaats vind. Een voorbeeld van een ziekte waarbij cortical atrophy optreed is dementia.
Duration of ketamine use was negatively correlated with gray matter volume in bilateral frontal cortex, whereas the estimated total lifetime ketamine consumption was negatively correlated with gray matter volume in left superior frontal gyrus.

Als ik het goed begrijp word hier gesteld dat er een negatieve correlatie is vastgesteld tussen duur van Ketamine gebruik en grey matter in de bilaterale frontale cortex. Dit houd in dat naarmate de duur toeneemt van Ketamine gebruik er een reductie in grey matter word waargenomen in de bilaterale frontale cortex (aan beiden hersenhelften van de frontale cortex). De negatieve correlatie tussen estimated total lifetime ketamine consumption en de linker superior frontal gyrus stelt dat naarmate de totale consumptie van ketamine toeneemt er een reductie van grey matter word vastgesteld in de linker superior frontal gyrus.
Conclusion: We have demonstrated a reduction in frontal gray matter volume in patients after chronic ketamine use. The link between frontal gray matter attenuation and the duration of ketamine use and cumulative doses of ketamine perhaps suggests a dose-dependent effect of long-term use of the drug.

Drugs misbruiken is gewoon vragen om problemen...
Maar nooit geweten dat een lage dosis keta de hersenen beschermt

Op zich moeten cognitieve achteruitgang en hersenschade ook hand in hand gaan. Hersenschade waar je geen gevolgen van ondervindt (op de korte dan wel lange termijn) hoef je misschien niet eens zo erg te vinden; aan de andere kant vindt cognitieve achteruitgang altijd zijn oorzaak in het brein en is dus altijd een bepaalde vorm van hersenschade.
Dat dit echter zorgwekkend is, daar ben ik het wel mee eens, al had ik wel verwacht dat heftig ketaminemisbruik zo zijn schadelijke gevolgen zou hebben. Ik meende echter toen ik het onderzoek gisteravond - weliswaar in wause status - doorlas, dat dit volgens mij niet over Olney's lesions gaat, maar over een andere vorm van hersenschade; de bevindingen van het artikel kwamen in ieder geval niet helemaal overeen met OL, geloof ik. Heb nu geen tijd om het na te checken, dus weet het niet zeker.

Ik heb het idee dat we misschien verschillende interpretaties hanteren van wat invloeden op cognitie inhouden?

Dat klopt maar beperkingen in cognitie zijn niet exclusief verbonden aan hersenschade. Medicatie en drugs kunnen een namelijk een tijdelijk effect hebben op cognitie iets wat niet per definitie resulteert in hersenschade. Dat hersenschade kan resulteren in cognitieve achteruitgang hangt ook sterk af van het gebied waarin deze schade optreed en of we spreken over een blijvende achteruitgang. Daarnaast heb je ook te maken met neuroplasticity deze ondervangt namelijk verschillende invloeden inclusief hersenschade. In hoeverre je hersenen in staat zullen zijn om deze schade te ondervangen hangt af van vele factoren. Op deze pagina staan een aantal voorbeelden van neuroplasticity.

aan de andere kant vindt cognitieve achteruitgang altijd zijn oorzaak in het brein en is dus altijd een bepaalde vorm van hersenschade.

Heb je ook al overwogen dat modulatie van neurotransmitters een invloed kunnen hebben op cognitie? Erfelijkheid is verder ook bepalend, net als het hebben van een psychische aandoening. Niet al deze gaan gepaard met hersenschade.

Ik ben benieuwd wat je bevindingen zijn over de studie mocht je er nog op komen.

In het artikel word het nergens vermeld of geciteerd dat klopt, of er daadwerkelijk Olney's lesion aanwezig zijn is mij dan ook niet helemaal duidelijk. Ik sluit het echter niet uit, echter ik heb de eerste post een beetje aangepast om deze onzekerheid te reflecteren. Volgens mij word het begrip Olney's Lesions ook niet meer gebruikt in recent medisch onderzoek (als het überhaupt ooit gebruikt werd).

Wat de studie voornamelijk in kaart brengt is in welke gebieden er schade vastgesteld word bij de proefpersonen. Het precieze mechanisme van schade is dan ook nog niet helemaal duidelijk. Wat wel is gebleken is dat er overeenkomst is tussen de bevonden schade en dierenmodellen en menselijke cel studies. Een degelijke indicatie voor verdere studie.

Het hangt ervanaf hoe je 'hersenschade' definiëert. Van zuiver biologisch sloopwerk hoeft inderdaad geen sprake te zijn. De zin van het onderscheid 'hersenschade' <-> 'biochemische veranderingen' ontgaat mij echter een beetje zodra er negatieve gevolgen aan te wijzen zijn :P Strikt gesproken heb je echter zeker gelijk.
Wat betreft Olney's Lesions: kan toch goed zijn dat dit hetzelfde is. Was behoorlijk naar de klote vannacht dus geloof dat ik e.e.a. verkeerd heb geïnterpreteerd. Weet het echter alsnog niet helemaal zeker en kan ook niet zeggen dat ik fris genoeg ben om het helemaal tot in detail uit te pluizen :#:

Abnormalities in White Matter Microstructure Associated with Chronic Ketamine Use, Neuropsychopharmacology (2014) 39, 329–338;
Ketamine is an N-methyl-D-aspartate (NMDA) receptor antagonist that has been found to induce schizophrenia-type symptoms in humans and is a potent and fast-acting antidepressant. It is also a relatively widespread drug of abuse, particularly in China and the UK. Acute administration has been well characterized, but the effect of extended periods of ketamine use—on brain structure in humans—remains poorly understood.

We measured indices of white matter microstructural integrity and connectivity in the brain of 16 ketamine users and 16 poly-drug-using controls, and we used probabilistic tractography to quantify changes in corticosubcortical connectivity associated with ketamine use. We found a reduction in the axial diffusivity profile of white matter in a right hemisphere network of white matter regions in ketamine users compared with controls. Within the ketamine-user group, we found a significant positive association between the connectivity profile between the caudate nucleus and the lateral prefrontal cortex and dissociative experiences.

These findings suggest that chronic ketamine use may be associated with widespread disruption of white matter integrity, and white matter pathways between subcortical and prefrontal cortical areas may in part predict individual differences in dissociative experiences due to ketamine use.

Olney's Lesions heb je niet zomaar te pakken hoor, dan hebben we het wel over langdurig misbruik va dissiociatieven. Het verschilt per middel maar om er een paar te noemen voor de mensen die geen zin hebben in lange wetenschappelijke stukken lezen:

Van hoof naar laag risico:

-PCP
-DXM
-Ketamine
-NO2

Wat gevaarlijker is bij keta is schade aan je blaas, dit kan snel optreden als je er gevoelig voor bent. Een grammetje per één of twee weken, maximaal 20 keer per jaar is in principe veilig.

Olney's Lesions have not yet been proven or disproven to manifest in humans. No tests have been conducted to test the validity of post-dissociative development of vacuolization in human brain tissue, and critics claim that animal testing is not a reliable predictor of the effects of dissociative substances on humans:

Ik heb zelf geen recentere informatie kunnen vinden over de Olney's lesions hypothesis. Ook ben ik geen nieuwe details in Ketamine literatuur tegengekomen hierover. Wellicht worden nu andere benamingen gebruikt voor aspecten die overeen komen met Olney's bevindingen?

Uit verschillende onderzoeken is nu wel naar voren gekomen dat Ketamine veranderingen veroorzaakt in de hersenen. Op welke wijze dit zich gaat manifesteren is nog steeds de vraag daarvoor is het academisch onderzoek nog te ver in de begin stadias. Ketamine als recreatieve drug heeft pas een echte grote opkomst gemaakt in de afgelopen paar jaren. Onderzoek in vergelijking met MDMA naar de schadelijkheid van recreatief gebruik is dan ook nog in de begin stadia. Ook zal er net als bij MDMA soortgelijke validiteit problematiek een rol spelen bij Ketamine onderzoek. Denk bijvoorbeeld aan het gebruik van meerdere drugs bij proefpersonen die het resultaat kunnen vertekenen.

Deze conclusie kun je niet zomaar trekken aangezien bewijs hiervoor ontbreekt. De onderzoeken die tot dusver zijn gedaan omvatten wellicht gebruikers in het uiterst van het gebruik-spectrum maar zolang er geen onderzoeken zijn gedaan met sporadische gebruikers bestaat de kans nog steeds dat deze effecten cummulatief over tijd kunnen zijn. Ongeacht of je dus lange pauzes inlast.

Ketamine, a non-competitive N-methyl-D-aspartate receptor antagonist, is rising in popularity as a drug of abuse. Preliminary evidence suggests that chronic, heavy ketamine use may have profound effects on spatial memory but the mechanism of these deficits is as yet unclear.

This study aimed to examine the neural mechanism by which heavy ketamine use impairs spatial memory processing. In a sample of 11 frequent ketamine users and 15 polydrug controls, matched for IQ, age and years in education. We used fMRI utilising an ROI approach to examine the neural activity of three regions known to support successful navigation; the hippocampus, parahippocampal gyrus and the caudate nucleus during a virtual reality task of spatial memory.

Frequent ketamine users displayed spatial memory deficits, accompanied by and related to, reduced activation in both the right hippocampus and left parahippocampal gyrus during navigation from memory, and in the left caudate during memory updating, compared to controls. Ketamine users also exhibited schizotypal and dissociative symptoms that were related to hippocampal activation. Impairments in spatial memory observed in ketamine users are related to changes in medial temporal lobe activation. Disrupted medial temporal lobe function may be a consequence of chronic ketamine abuse and may relate to schizophrenia-like symptomatology observed in ketamine users.

Dit maakt me dan toch voorzichtig, al ben ik zelf ondertussen niet meer 'developing':
